YTD Current financial statement (not older than 90 days)

Personal Information

Personal financial statement (form attached)
Last 3 years personal tax returns, including all forms, schedules, W2 forms, and K-1 statements
Bio form (form attached) and/or resume
Most recent pay stub or YTD Income through March 2015

**If request includes construction,

Provide a copy of the construction contract (or contractor’s estimate of cost), and a detailed breakdown of total project cost, including property cost, construction hard costs, construction soft costs, equipment, etc.

**If request includes real estate purchase

Include an executed copy of the purchase contract or letter of intent

Copies of any available appraisal reports, environmental reports, and/or survey
